One of the more intricate challenges faced within the realm of medical documentation in Germany is the art of code-switching between professional and layman’s language. This linguistic dexterity is especially pertinent when dealing with medical history and the process of conveying the patient’s information. Initially, information gathered from the patient, often presented in layman’s terms using vernacular expressions, must be adeptly “translated” into the register and lexicon of professional medical language. This translation necessitates the use of the highest level of Greek- and Latin-based clinical terminology, intricately embedded within phraseo-terminological units specific to the respective medical field. Furthermore, this linguistic interplay extends to epicrisis and assessment, where the course of the disease and the administered treatment are briefly summarised for another physician. In this context, the use of highly specialised professional terminology becomes inevitable. However, the complexity of medical documentation transcends professional jargon; it also involves the vital tasks of providing instructions and detailing the subsequent steps for the patient in layman’s language, employing vernacular terms to ensure patient comprehension. This intricate process is not exclusive to non-native speakers of the German language; it presents a challenge even for native German speakers, not least because documentation skills are typically acquired in clinical practice rather than being explicitly taught at universities (Bechmann, 2017).
